Patient Concerns / Grievances Process
Inland Home Health Providers strives to ensure quality home healthcare for patients. Our objective is to provide top quality health care service that is consistent with our mission statement. If for any reason you, as our patient, have any concerns or grievances resulting from the service provided, we would appreciate the opportunity to assist you to resolve any problems that may arise.
To initiate the process, please call Inland Home Health Providers at 909-948-8731.
Ask to speak with the Director of Patient Care Services and identify your concerns.
The Director of Patient Care Services will investigate your concern and within three (3) days from the date you called, the Director will render determination and resolution we hope to satisfy your concern.
- If the Director of Patient Care Services is not able to provide you the proper resolution to your concern or grievance, we encourage you to contact the Administrator.
The Administrator of Inland Home Health Providers will provide you the best possible resolution within five (5) days.
